1) Go to http://www.azhpa.org.

2) Scroll down to the Donate button on the left hand side. Click it.

3) Type in an amount and logon to PayPal or if you don't have a PayPal account,
click Continue above the credit card symbol, enter your name and credit card
info, click Continue

4) Click "Special instructions to the seller", enter “Mark Knight."
