Please back file all incidents that have happened in the last
three months. These are anonymous, but the date and location are sent to
directors. So I am going to know if you're being honest. Again, if it deviated
from a normal flight plan and could have resulted in an accident, it was an

first two steps and you can skip the third step unless there was property damage
to somebody else. It does not have to be your incident, so please report the ones you see. What's an incident? Anything which deviated from a normal flight plan that could
have resulted in an accident. Botched launch that the wing went in the trees or
got wrapped around the ramp? Incident. Landing where the pilot came up short and
went into the swamp? Incident. HG pilot "whacks" on landing and broke a tube?
Incident. PG pilot got blown back and tossed reserve? Incident! Pilot had
a bad launch on tow and broke the weak link? Incident.

to fill out so that: all USHPA members can learn from what happened and fly more
safely; to make sure pilots and the organization are legally protected; and to
ensure the office is informed. If you provide your email address and check the appropriate boxes, you will be
