USHPA SOP 03-16.05

A. All reports submitted using an on-line accident report forms are
automatically forwarded to USHPA legal counsel. USHPA retains no copy of
reports.

B. All reports received by the USHPA office, the Chair of Safety and Training or
the Accident Review Board Chairs are immediately forwarded to the USHPA legal
counsel.

Never to be seen again, apparently.

So tell me again why you would want to send in an accident report to the USHPA
especially on line?
