The HGFA accident database

We had/have very similar issues with our online reporting system
in Australia. The old system had the complete database falling into "a black
hole". To compound this, pilots seem to not bother with sending in reports in
the hard copy form either. Maybe they see no benefit to a report that no one
learns from.


The new HGFA Operations manager John Olliff, has worked toward the new online
system that is called IRIS. This new system is on trial in Australia for one
year from its release date which is very close. The IRIS system allows for easy
online reporting by pilots and third parties, i.e. witnesses. It has three
access tiers which enables Instructors, club SSO's to review the accident
information and post their analysis. Statistics for identifying trends etc can
also be pulled from the site (names removed of course) and used to provide
positive learning for club pilots.


The IRIS system satisfies CASA requirements and also frees up valuable staff
resources in the HGFA office.


Perhaps you guys could look at something similar as your system seems to be
failing in the same way as ours has.


The next twelve months will be the litmus test for IRIS and for the HGFA/CASA
relationship.
